SHIFT SUPERVISOR / CHARGE NURSE

JOB DESCRIPTION

POSITION PURPOSE AND SUMMARY

As charge nurse you are delegated to the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

            RESPONSIBLE TO:              Director of Nursing

        QUALIFICATIONS:             1. Graduate of an accredited school of Nursing, Current Licensure in State of CA, BLS / PALS Proficiency

The Charge Nurse/Supervisor uses an interdisciplinary approach with residents, their families, support departments, etc., to adequately plan for the needs of patients.  They perform specific shift responsibilities.

BASIC DUTIES AND RESPONSIBILITIES

1. Count all controlled substances at the beginning and end of each shift.
2. Make assignments at the beginning and end of each shift.
3. Receive shift report 7:00am, 3:00pm, 11:00 pm from off going shift.
4. Verify that keys have been turned in by outgoing shift.
5. Evaluates resident health status as well as health care services by making frequent resident rounds. Documents informative entries in the medical record (chart and care plan).
6. Check MARS for meds to be given; treatments to be done on own assigned patients.
7. Administer meds and treatments as prescribed by physician and document patient’s response to medications and treatments on own patients.
8. Assists Nurses as needed to assure the delivery of timely resident care.
9. Provides direct supervision and evaluation of work performance of Nurses C.N.A.’s
10. Provide nursing care for assigned patients.
11. Communicates with physicians’ changes of condition or updates of patient’s condition.
12. Rounds with physicians as appropriate.
13. Monitor ancillary departments, i.e. lab and radiology for timely completion of test, assessment of results and MD notification.
14. Oversee and assist in lunch relief.
15. Provide relief for nursing staff as necessary.
16. Attends in-service education and staff meetings.
17. Assures medications refill are ordered as necessary
18. Serves as a resource for all staff including RNs, LVNs, CNAs, & RTs
19. Assures 24-hour nursing report is completed for their shift.
20. Facilitate disposal of outdated meds.
21. Ensure maintenance of crash cart readiness.
22. Ensure emergency preparedness by promoting active participation in drills etc. by all staff.
23. Reviews audits done by pharmacy and medical records and supervises staff in taking appropriate action to correct deficiencies in a timely manner.
24. Verify fax sent on STAT meds needed from pharmacy.  Receive and verify any meds delivered by pharmacy.
25. Ensures compliance with infection control practices by all personnel under his/her supervision.
26. Reviews and completes Dietary recommendations of nutritional assessments.
27. Responsible for accurate observations, evaluation and report of resident symptoms.
28. Assists nursing personnel with treatments/emergencies and provides direct nursing care to critically ill residents as needed.
29. Assist with education of residents, families and personnel as needed.
30. Night Shift Charge Nurse or designee reviews all orders of assigned patients received from previous 24 hr. and audits for accurate and complete noting of orders.  This includes treatment/medication transcriptions, care planning, nursing notes, etc.
31. Coordinates nursing care with ancillary services.
32. Protects the resident, his/her privacy and property.
33. Check MARs, TARs, UDAs.
34. Take sick calls. Attempt to get replacement.  May delegate this to Unit Clerks if available.
35. Performs special projects on an ‘as needed basis’.
36. Performs any other duties as may be requested or assigned by DON.

EDUCATION REQUIREMENTS

Must possess as minimum, a Nursing Degree from an accredited college or university or be a graduate of an approved RN program. Minimum one year of nursing experience, pediatrics preferred.

QUALIFICATIONS

• Must be able to read, write, speak, and understand the English language.
• Able to make independent decisions when circumstances warrant such action
• Possess the ability to deal tactfully with personnel, residents’ family members and visitors to facility
• Knowledgeable of nursing and medical practices and procedures as well as laws, regulations and guidelines that pertain to long term care
• Possess leadership and supervisory ability and willingness to work harmoniously with professional and non-professional personnel
• Possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures that are necessary for quality care
• Must have patience, tact, a cheerful disposition and enthusiasm as well as a willingness to handle conflict
• Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices
• Be able to relate appropriate information concerning a patient’s conditionPHYSICAL AND SENSORY REQUIREMENTS

1. Be able to move intermittently throughout the workday.
2. Be able to cope with mental and emotional stress of the position.
3. Be in good health and demonstrate emotional stability.
4. Able to relate to and work with the ill, disabled, emotionally upset and at times hostile people within the facility.
5. Must be able to lift, push, pull and move a minimum of 40 lbs.
6. Must be able to assist in evacuation of residents during an emergency.
7. Must be able to lead a Code Team.
